OPTIMIZE YOUR MAIL DELIVERY
DO
Use a standard-sized envelope, such as a No. 10, 6x9 or 9.5x12.5.
DON'T
Use an uncommon envelope, such as a very large envelope (12" x 15"), or a heavy cardstock envelope. Uncommon envelopes typically take 3-5 days longer to be delivered.
DO
Let us know where to mail your documents either by providing written instructions or by Including a self-addressed return envelope.
DON'T
Assume we'll know where to mail your documents. While we're pretty good at guessing where to mail documents, sometimes we'll guess incorrectly if the address in box 1 differs from the address in box 3.
DO
Type the address label or print neatly.
DON'T
Handwrite the address illegibly. (While most handwritten envelopes will take 5-7 days to arrive, some will take as long as two weeks to arrive.)

Note regarding the last business day of the year (New Year's Eve): Typically, we do not receive mail or overnight letters on the last business day of the year.

Common reasons for forms to be rejected:
1. A county name is missing in Box 1.
2. A street address is missing in Box 1; or a P.O. Box is written in Box 1.
3. If an application has more than two fictitious business names, the additional business names above two are written on another Fictitious Business Name Statement, rather than on Sacramento County's SHEET 2 (ADDITIONAL INFORMATION) page.
4. Boxes 3 and 4 don't match. If an individual's name is written in Box 3, then "an individual" in Box 4 should be checked. If an LLC name is written in Box 3, then "Limited Liability Company" in Box 4 should be checked.
5. Form is unsigned.
6. For corporations and LLCs, an unacceptable title (such as "Owner") is written in Box 6 under the signature. A list of acceptable titles is printed on page 2 of the Fictitious Business Name Statement.
7. The incorrect filing fee is included.
8. The filing fee check is made out to the incorrect payee. Checks should be made payable to Sacramento County or Sacramento County Clerk.
